How Swiss health insurance premiums are set

Basic insurance (KVG) covers exactly the same treatments at every insurer. The law defines the benefits, so the only things that change between offers are the price, the insurance model and the service. Each insurer files its premiums with the Federal Office of Public Health every year, and may only charge what is approved. That is why an identical product can cost hundreds of francs more at one insurer than another, and why comparing on price alone is reasonable here in a way it would not be for a car or a house.

Why your postcode decides the price

Premiums are approved per canton and, in larger cantons, per premium region. There are up to four regions in a canton, reflecting how much healthcare actually costs there. Your commune sits in exactly one of them, so two families 20 minutes apart can face different prices for the same policy. This tool resolves your postcode to its official premium region first, then shows only the premiums that genuinely apply to you.

Deductible, excess and what you actually pay

You pay your chosen deductible in full each year before the insurer contributes. Above it you pay a 10% excess, capped at CHF 700 a year for adults and CHF 350 for children. A higher deductible always means a lower premium, so the question is whether the premium saving is bigger than the extra you would pay in a year you need care. The four insurance models

Standard gives you a free choice of doctor. The family doctor model asks you to see your chosen doctor first. HMO models use a specific health centre network. Other models cover telemedicine and similar restricted-choice options. Restricting your choice lowers the premium, often substantially. The medical benefits stay identical in all four, because the law defines them.

Changing insurer, and the deadlines

You can switch basic insurance to another insurer at the end of a calendar year by giving notice that reaches them by 30 November. Insurers must accept every applicant for basic insurance regardless of age or health. Supplementary insurance is different: it is optional, insurers can refuse you, and it is worth settling any new supplementary cover before you cancel the old one.

What this comparison does not include

This is basic insurance only, at the approved price. It does not rank insurers on service, claim handling or waiting times, and it does not cover supplementary insurance, which is not standardised and not comparable on price alone. We also earn nothing from your choice: there is no commission and no insurer is hidden, which is not true of every comparison site.

Questions about comparing Swiss health insurance

Is basic insurance really identical at every insurer?

Yes. The benefits of basic insurance (KVG) are defined by federal law and are the same everywhere. What differs is the premium, the insurance model and the service you get.

Where does this data come from?

From the premiums the Federal Office of Public Health approves and publishes each year as open data, plus the official commune-to-premium-region map from priminfo.admin.ch. It is the same source the federal premium calculator uses.

Why is my neighbour's premium different?

Age group, deductible, insurance model and whether accident cover is included all change the price. If they live in a different commune, they may also be in a different premium region.

Should I exclude accident cover?

If you are employed, your employer insures you against accidents by law, so accident cover in your health insurance is usually duplicate. This tool lets you compare both ways so you can see the difference. Whether it applies to you depends on your employment, so check your contract.

Do children pay less?

Yes, and many insurers reduce the premium further for additional children. Pick which child you are pricing to see the rate that applies.

When can I change insurer?

For basic insurance, at the end of the calendar year, with notice reaching your current insurer by 30 November. A change of deductible or model also takes effect at the start of a year.
